- Other names or titles (optional item). If you have a pseudonym, or the brand is known under several names, an abbreviated version, indicate this.

At the end you need to click "Submit". You can fill out an application in Russian.
Filling out the application in English, you will not speed up its consideration and will not affect Instagram's decision on verification.
You can't get a blue tick on Instagram without a request. The social network does not automatically give it out, even if you are a very famous person.
